Include state and city in the address section in the application form. Many candidates fill just street and you can't tell which city and state there are.

Agree. Maybe it can be an official address field where they have to fill in actual zipcode etc. We have started just asking for zipcode on our application questionnaire as the address field is so unhelpful.
